In production, this table should be auto-generated from the
doctor master, planned frequency, and DCR visit data. When you
submit a DCR with a visit for a doctor:
• “Visits this month” increments
• “Remaining visits” updates
• Status moves from “Overdue” → “On track” → “Completed”
